Here is one that is not necessarily classic, but before Sally Field was offered her oscar-winning role in/as Norma Rae, Jane Fonda and Jill Clayburgh were offered the role. Ironically, all three were nominated for the oscar that year. You have to imagine what Jane and Jill were thinking. Personally, had I been offered Norma Rae, I would have took it!!!!!!
One that instantly comes to mind is that Debra Winger was going to play the title role in Peggy Sue got Married, but had to pull out because of back injury, only then did Kathleen Turner get the role.
BTW, a back injury is also what kept Colbert out of All About Eve.....
