my grandma lives in Staten Island and i always referred to her living in "the city" the island is long island
As a non NYerker, I think of -the city- as Manhattan.
The city to me is any of the 5 boroughs (including the two that are on Long Island.) Just like "The Island" is Long Island (including Brooklyn and Queens) because I'm from "the Island."

As a non-New Yorker, I tend to think of it as Manhattan.
I happen to like New York...
To me, it means Manhattan. If I'm going to the Bronx, I say "The Bronx"
As a NYer in suburbia, I consider any of the boroughs the city.
Then again, my friend from California, whenever I refer to "the city" thinks I am talking about Sacramento...
I think of all 5 boroughs...HA, and I love this discussion of "the island" because I actually have heard someone call Manhattan "the island" and in my head I was, "excuse me? while Manhattan IS an island..."
definately just Manhattan, but only because i'm rarely (if ever) in the other boroughs
