I think the best entrance, although not her initial entrance into the show, was Carla in Nine, with Jane Krakowski descending from the rafters in a towel.

I really don't remember the man playing Audrey 2 being onstage the entire time, even before the shows starts.

In Peter Hall's original staging of AMADEUS, Salieri's wheelchair was sitting center stage with its back to the audience when they opened the house 1/2 hour before curtain. It looked like somebody was sitting there, but you weren't sure. When the lights began to fade at the openning, Ian McKellan yelled out "Mozart!", and you realized he'd been sitting there all along.
