I have noticed some photos people have posted of fan mail they've received back, the actor/actress also sent back a questionnaire to them. Are these questionnaires usually self-made? Or is there a template/site to get them? Thanks for the help!

You have to send them the questionare, and there is no official template, it is just a sheet of some questions you want them to answer, so you chose the questions.:)
