So say an understudy for a lead, can they get nominated, or a featured actor, or a swing member for one role they play?

Only if they played the role on opening night. (Or some exception is made as in the case of Larry Kert, who understudied Dean Jones on the opening night of COMPANY, but took over the role permanently shortly thereafter. AFAIK, that is the only time that particular exception was made.)
