Has anyone ever fallen off a balcony in the theatre or even like an actor falling off the cat walk or grid....dumb question but whatever!ha!

You'll probably get more answers if you change the title to be more specific. Adrian Bailey is the most famous case I can think of, falling through a trap door in a boat 40 feet to the stage at The Little Mermaid years ago. He survived, but suffers from the injuries to this day. The guy from Spiderman whose name escapes me. Nancy Opel fell through a trap door in the stage during Fiddler, I think. According to wikipedia, Michael Berresse has twice ended up in orchestra pits, as have many other actors.
There are several cases of stagehands falling, as they are far more likely to be on catwalks or grids. The best known of these was a fall a stagehand took at Radio City Music Hall. He is now a paraplegic.
Also, there's Bantcho Bantchevsky, who deliberately fell to his death at the intermission of
Verdi's MacBeth in 1988
