What's the policy? How much is it, what time do they get on sale, how early do you have to be there, etc.?

I did it in August. They go on sale when the box office opens. I got in line around 9am and was 5th in line. (they only sell 10 standing spots) 10 minutes later, the line was full.

They must have changed the policy from August then because now they actually sell them 2 hours prior to the performance and usually there are about 12 of them availible, 1 per person...for $26.25

i got there at like 10 and JUST made standing room.
the view is still good, but it's really what it means.. standing room. no chairs or anything, and the performance i was at was sold out [and i'm sure most of the performances are] so there were no empty chairs. my legs got tired after a while but it happens.
the view is pretty good, you can't see the tops of heads on the balcony part, and you have to shift a lot at certain points to see everything, but for $26 it was fantastic.
