Most people don't realize that film/TV sets are a lot like a construction site and while the union cre ARE well trained professionals all it takes is for a screw to crack & sheer and whoopsie!
One memory I have is of a steady cam rig that was being supported on a stand( correctly i might add) when suddenly KA thunk the camera rig fell off narrowly missing the foot of one of the makeup women ( did graze her leg on the way down only slight abrasion) Camera are OBSSESIVE about their equipment and its care and feeding. There was a bout a 3 sec freeze while everyone absorbed what happened and then mad hysteria! I was even asked to help & had to explain i only help hurt "people" . There were calls to Panasonic in LA , screaming- frantic arrangements to get new equip & repairs to what was damaged. and That was egzacly what had happened- a screw holding the camera into the rig had gradually been stressed and finally sheered.
Remember that old poem "for want of a nail..."?
