Replacement and Understudies Question

How are replacements rehearsed for shows? I'm assuming that the original directors don't come back to direct replacments in shows. Aka: I'm assuming Mike Nicolas did not come around to Spamalot again to direct Lauren Kennedy etc. So who does it? How does the replacement process work, rehearsal wise?

Also when do the understudies rehearse? Are they around when the actor who is actually cast in the role learns their blocking and whatnot? Or do they have sperate rehearsals? Thanks.
