Hair and eye color (if for film)

Contact information: email, phone (NOT your mailing address)

Credits: play title, role, place performed, and director (if its a notable person)

Before you get snarky, lightguy, note that Ashley posted that over a year ago. Now try and be productive, eh?

The link here is for theatre students, but I would suggest looking at any theatre actor's website and seeing if they have a posted resume. That should give you a start.
