Does the cast usually stay the same? Or are there usually changes. RENT pretty much stayed the same, didn't it?

It depends on the show (and on the wishes of the producers).

I'm not sure if Rent had the same cast for NYTW. I know for the original reading of it at NYTW, it had the following cast:

Roger Davis - Tony Hoylen
Mark Cohen - Anthony Rapp
Tom Collins - Pat Briggs
Benjamin Coffin III - Michael Potts
Joanne Jefferson - Shelly Dickinson
Angel Dumott Schunard - Mark Setlock

I know that at least the main cast, and I would imagine the supporting cast though I'm not postive, all remained the same in Rent.

The cast of the last Rent NYTW cast and the original Broadway cast was pretty much the same.

95% time, casts remain completely in tact for Off-Broadway to Broadway transfers.

It's very rare that there is a cast change from Off-Broadway to Broadway since the reason that the show is moving in the first place is because of its success, so why tamper with a sure thing. When changes do occur, they tend to happen if there is long break between the move and an actor becomes unavailable. I'm sure that there have been cases when an actor or actress has been replaced but that would be the exception and not the rule.
