Best is so much harder, happily. I'd pick (partly for sentimental reasons): Rent OBC Ragtime OBC Chicago revival original cast Special mention to The Book of Mormon which...it's too soon to include it, but I LOVED it. And Patti LuPone Gypsy which juuuust missed and would probably knock Ragtime off the list.
Worst: Lord of the Rings in Toronto Brooklyn OBC Martin Guerre in London Dishonorable mention to the off-off-Broadway musical Imelda which was actually worse than any of these but also had less to work with, so I found its failures less spectacular.
