To establish my pseudo-credibility... I spent many a year as head chorister and cantor at St. Marks Episcopal. (And was at the time the youngest Episcopan Dean's Award recipient - for Choral Music - in NY State).
As I understand it...
Reverend Mr./Ms. or
Rector is a pastoral leader, or head of a local parish.
The Very Reverend is a Dean, or head of a cathedral or seat of the region.
The Right Reverend or
Diocesan Bishop is the head of the diocese.
So, I believe that we now have...
