Not a movie but a particular moment that still sticks with me to this very day.

Ha, that part used to terrify me as well.
The boat sequence in WILLY WONKA AND THE CHOCOLATE FACTORY...eek, a chicken getting its head cut off!
Aforementioned SNOW WHITE and the forest.
And LITTLE HOUSE ON THE PRAIRIE. Yes, you heard me correctly. For a show that was so frequently about home-y, family, Christian values, that show went to some dark-ass places! There was one Halloween episode where Laura sees Mr. Olsen chop off a mannequinn's head with a fencing foil and she thinks he has murdered Mrs. Olsen. This sets Laura into a series of nightmares about chopped off heads that scared the bejesus out of me.
Later, there was an episode about dull old Albert falling for a new girl who was made fun of because she was "developed". She would get stalked and attacked, HALLOWEEN-style by a guy in a scary mask who would rape her. The stalker turns out to be her father. I remember the episode ending with her dying in Albert's arms. Ah, LITTLE HOUSE!
Since we're moving onto tv shows, there was an episode of WEBSTER (after he burned down the apartment and they moved into the house with those other people) where Webster saw a girl in a rocking chair when he peered into an old room upstairs. Turns out it was only a doll but it used to scare the shlt out of me.
If we're talking about TV shows, there was this one episode of one of the newer OUTER LIMITS series that REALLY freaked me out. I knew the name at one point, but I can't remember it right now. It was about this scientist who created this robot to replace his dead son. I watched, maybe, five minutes of it but those five minutes haunted me for YEARS. In the scene, the scientist came home and saw the robot and the robot said "I'm petting the cat!" or something to that effect, and the camera pans down to show the cat dismembered and bleeding. WOW, I couldn't sleep for WEEKS after that.
There were episodes of Unsolved Mysteries and America's Most Wanted that made me want to hide under the covers and erase from my brain because of how intense and creepy the reenactments were.
Re TV, what child of the 80s can forget the classic "Indian Burial Ground" episode of Punky Brewster! Man, oh man. http://www.cracked.com/article_19401_5-inexplicably-horrifying-episodes-classic-comedies.html
But I admit, even relatively tame things terrified me--like the weird Twilight Zone episode of my fave show, The Facts of Life.

When I was a kid, the commercials for the movie of Tom Tryon ("Colin, where isssss the baby?") scared me pretty bad.

'Alice in Wonderland' has been mentioned a couple of times. I cannot forgot how the 1985 made for TV adaptation of Wonderland/Through the Looking Glass with the Jabberwocky scared me.
