"this underwear" as you are referring to one underwear. Unless, since one underwear is referred to as a PAIR of underwear (why???) than it might be "these underwear", but really you're only wearing one, are you not?. oy, now I've confused myself.

According to the dictionary, an article of clothing is referred to as a "pair" because it is made from two corresponding pieces of fabric. I just looked it up ...

I would have said: I really like this underwear. But it's I really like these jeans... so I'm confused.

some of that confusion comes from the what is "paired"...for instance, "jeans" has a plural connotation, even if "pair" refers to the ancient practice of two separate fabric leg coverings paired together for one garment, like "hose" or "pants" or "trousers". If the word "knickers" was used instead of "underwear" (the latter having no final "s") it seems easier.

And clothing is not all that has that weird etymology---there is also, at least, the case of "scissors" and "shears" which were once separate blades, then joined into a pair for easiest use.

Can you tell i'm bored?

It is this, because "underwear" implies the phrase "pair of," and "pair" is singular.

No, it doesn't. That would also mean that "jeans" implies "pair of," which it most certainly does not. You can't "imply a word...it will change what word makes the sentence correct.

However, it should be "this underwear" because you are referring to the "wear" as one thing as opposed to "undergarments," which refers to the wear as multiple, separate garments. Make sense?
