Its left to your opinion...Was it the priest, the farmhand or a miracle. In the play Dr also says that she stopped smoking, and began having her mensturation again. Leaving the audience to believe that a miracle has happened.

I believe it all went down hill when she began having her mensuration again... whatever THAT is.

The paternity of the baby is left deliberately ambiguous. The farmhand? God? Who knows.
