Question about the ACTUAL Tony Award

So now that Angela Lansbury has 5 Tony Awards, does her Tony from 1966 resemble her Tony from 2009? The Oscars have had a few changes, but have looked basically the same since the late 1940's.
I have seen pictures from the early Tony Awards, and it looked like they received something like a medal (without the ribbon).

Also, do the winners receive a name plate that goes on the award?

The initial award Angela Lansbury received was the Tony medallion, the main part of the award now, but without the black pedestal that was introduced later. I read that her first Tony was lost in a house fire so maybe it was replaced by the current model.
