First time I saw Tarzan, in the opening number when the cheetah is suppose to attack and kill Tarzan's parents, the tree house is suppose to rise up and then when he "kills them" they are pulled off the structure and back into the wings but a rig. When I saw it through, something happened and the tree house never rose up so when it was time for the cheetah to do the thing, he just kinda came down, looked at them and the parents slowly walked backwards off the stage awkwardly.

I was at a preview performance of the current Miss Saigon revival when the helicopter wouldn't make its final ascent. They had to clear the stage and stop the show for about 10 minutes or so. Definitely a memorable night at the theatre!

I saw a preview performance of Misery a few years ago and at one point, the main house set stopped rotating. The rotation was pretty integral in allowing Bruce Willis to move about the stage so they just put the curtain down for 5-10 mins before they had it up and running again.
