I've just been working out my personal statistics since commencing regular theatre-going in July 2006. So far in 2008, I'm averaging 8 trips a month, which is 0.6 up on 2007, and 4.2 up on 2006. (I haven't started going into location (city), location (theatre), or play/musical/other yet, but I already know what my most-visited theatre ever is. XD)
My monthly average for 2008 is pretty much mostly down to May, when I saw no fewer than sixteen shows (fifteen plays, one musical).

Actually, 'The Lover/Collection' and 'Single Spies' were double-bills, two one act plays presented in the same evening with an interval between, so if we wanted to be creative or literal, we could say I saw eighteen shows that month. A thought that makes me a little giddy. O_o
My previous best was fifteen in one month, back in March 2007, which saw a trip to New York and the Get Into London Theatre promotion. XD
Sooo... what's your monthly average, and what's the most number of shows you've seen in a single month?
I usually see 3 or 4 shows a month, but this past July (as in last month) I saw 6 shows.
Wicked, In The Heights, Spring Awakening, [title of show], A Catered Affair and Legally Blonde.

TonyVincent: if we're going to be THAT creative/liberal we could easily condense all eight down into two: the Richard II-Henry V tetralogy, and the Henry VI-Richard III tetralogy. Both times, it's essentially one continuous story told over four plays, and there's not even that much of a break between the two tetralogies. ;D
Although seeing as each part was at least three and a half hours, I'm keeping it as eight separate plays. My butt EARNED that right. But as they were all over one weekend in the same venue, we could accurately refer to it as a mere four trips to the theatre. Or one, with "going back for sleeps" in between. XD

Main reason for so many shows in the summer months is just due to not having school, and many family members come during December and November to see shows. Plus, May is my parent's birthday month, so they usually enjoy seeing shows.

8-10 avg. In a slow month 2-3. The most around 20.
I saw Les Miz 5 times in one weekend twice in 2003, and I might've seen Jersey Boys 4 or 5 times in one month last year, but I'm not sure.
I don't keep records, but I did spend the full month of June this year in NYC and saw 23 shows during the month (about half Broadway and half off-Broadway. But I doubt if that's my record.

I saw 27 shows last September, The Wedding Singer twice in Baltimore, Xanadu (on stage) and 24 shows (in a span of 8 days!) at the NYMF. You could techinally say I saw 28 if you count Margeret Cho's Sensual Woman, which I saw at the Zipper.
This September I am currently slated to see 26 shows, Ace at Signature Theater and 25 shows (in a span of 9 days!) at the NYMF.
On a vacation in London I saw 11 shows in 7 days, thanks to one West End theater having a Tuesday matinee in addition to Wednesdays and weekends and another having a Monday evening performance. Unfortunately, it's been so long that I can't remember all of the shows.
