Definitely "Sixth Sense." Also "The Others." Which reminds me--the TV movie "The Other" had a great twist.
Definitely! I bought "The Other" last year on DVD, and was surprised how much it still creeped me out.
Plus, it has Uta Hagen in a rare screen appearance!

"Showgirls" had a great twist ending in that I actually stayed to watch it.
I certainly didn't see that coming.
As far as surprise endings go, it really doesn't get any better than NEWHEART.

I must be tired, because I am sitting here giggling about Oliver's Twist ending.
I managed to see The Shape of Things live and unspoiled. I was definitely shocked.
I find that a lot of Chuck Palahniuk's books have great twists. Sure, Fight Club started a really bad trend (I actually loved the book's ambiguous ending) but Invisible Monsters and even Survivor had me as a reader.
Chinatown's plot twist, especially when you tack on that ending, really shocked me.
Two films where it was one of the few times I did not hear about the plot twist beforehand was The Game (it sort of works but can Sean Penn's character really be the guy behind it?) and Jacob's Ladder.
I remember knowing that the ending to The Mist would be different from its source material but, wow. Sort of Night of the Living Dead's ending (is Ben getting shot (mistakenly or malevolently) by possibly racist zombie hunters a plot twist?).
Who am I kidding? It's clearly Sleepaway Camp.

Sleepaway Camp was a real shock to me.
And the end of the original Saw was awesome. I know the series has become (understandably) maligned for ushering in torture porn and all that, but the original is an excellent puzzle of a movie(and in truth has very little gore - it's all suggestion).
Classic misdirection, he was there in front of you the entire time.
Seriously fellow horror fans, did any of you see that end coming?
I was surprised by a Spanish film from a few years ago called The Orphanage.
And if you like to be truly stunned by an ending then do yourselves a favor and go into Almodovar's new film The Skin I Live In as cold as possible. It's a doozie! (And disturbing as hell).
I just noticed no one mentioned Psycho! One of the all time great twists.

Special mention to Mulholland Drive. The entire movie is one big twist. I'm not sure any of it actually makes sense, but it's a fantastic movie that throws so many twists at you that you're never sure what's reality and what's not.

The ending to the latest FINAL DESTINATION film is one of my favorite "twists" in a very long time.
