DO NOT use toasted plain sliced white bread, it doesn't have the right texture and your bread pudding will end up tasting like toast soaked in milk. Use toasted croissants, toasted brioche, or toasted challah. Toasting the bread before hand won't make it crunchy, it just removes the excess moisture so the bread will soak up more of the custard mixture.
I have a great recipe that I got from the restaurant I used to work at, it was one of the signature desserts there, but it's kind of complicated and time consuming.
The one above sounds pretty good to me. And you could always melt some bittersweet chocolate and whisk it in to the custard if you want it to be even more orgasmic.
