From what I am able to research, the fat is real.
He gained about 30 pounds for his role in "The Informant."
Since his character also sports a moustache, I would assume this is Matt out of make-up.

Matt can do no wrong, I think he is my favorite actor of his generation. To this day I can't get over the fact he didn't win an Oscar (or get a nomination) for MR. RIPLEY.
