I think she did have ribs removed. That is inhumanly possible to have that small of waist without some work done. That's not attractive whatsoever.

It is possible to get a waist that small with a very tightly laced, heavily boned corset.
I adore corsets, but I draw the line at about a 24" waist. Anything smaller looks freakish.

If she's been waist training for a long time, so much that she's even gotten her natural waist to a disproportionate 21 inches, there's probably not an acute health risk in her corseting to 16 inches. Not riskless, but not certainly indicating rib removal or long term damage to the organs.
Yes, it's weird and fetishistic. Also, not really attractive. But I wouldn't worry too much about her health.
