Don't know if favorite is the right term, but Johnny Got His Gun ...
Silence of the Lambs, I don't know if that's the sort of "disturbing" you're going for.
Memento
All That Jazz
Finally saw "Henry: Portrait of a Serial Killer" last night and LOVED it! Great filmmaking (very cinema verite mid 80s...Miami Vice-ish.)
I thought the acting was remarkable. Tracy Arnold was great! (Her hairdo was tragic, however.)

Frankly, that icky little video of Sarah Palin on Saturday Night Live's Presidential Bash, the one where she says she'll take down NBC when she's elected, is the creepiest and most disturbing film of the year to me.
I didn't read the whole thread, but I agree with Requiem for a Dream. Also Boxing Helena.
Oldboy
I heard about the movie when it was talked about on Bravo's Scariest Movie Moments, and so I decided to rent it, and it was a really disturbing piece of cinema to watch.
I also hear that Will Smith and Steven Spielberg will be remaking Oldboy.
Pink Flamingos
Eraserhead
Rent (I was disturbed by Adam Pascal's hair)
Audition is soooo diturbing. It makes my stomach hurt.
You are joking about the Oldboy remake, right?...RIGHT?!!
Francis Ford Coppola's "lost, cult flick ", THE CONVERSATION, circa 1974, starring Gene Hackman. The last scene is like nothing you've ever seen on screen. The destruction force of paranoia.

not in any way a favorite, but "The Reflecting Skin" is rather disturbing.

My pick's gotta be "Bubble" from Steven Soderbergh.
As well as:
M
Freaks-(1932)
Hard Candy
The Devil's Rejects
Rosemary's Baby
"Seven" with Morgan Freeman and Brad Pitt.
The Vanishing (the original not the remake with the happier ending). The last three minutes of the original haunted me for months!

SECONDS - Probably, the best film Rock Hudson ever did. I remember it vividly even to this day.
When does Cruel's autobio pic come out?

Testament-not sure if this film is disturbing, or just depressing. TV movie about nuclear holocaust. Features a very young Kevin Costner, as I recall.
I agree about Salo. One of those films that after viewing, you wish you could erase the memory.
The Oldboy remake info is correct. Spielberg is producing.
I wonder if he'll do the whole trilogy?
Oh I have to agree about Seconds! One of the most terrifying endings I have seen in a long time!
I can't believe Teeth hasn't been mentioned yet!
I also found Akira had many disturbing scenes. Especially where Tetsuo's thinks his insides are falling out of his body.
