I think you should lay it all on the line:
Dear X: Your humming annoys the f-ing hell out of me. And while we're on the subject (then go on to list everything about her that annoys you). Tension in the office could be lots of fun.

If all else fails you can slap her face until she bleeds, kick her until she's dead and then say bad things about her mother.
Works like a charm.

You could, also, remember that it could always be worse. I have one coworker who clips her toenails and fingernails in her office. You can hear the snip, snip, snip. Another coworker, who is 4 years younger than me, acts like he's the president of the company and knows everything. Another coworker makes personal calls all day long but will be the first to tattle if someone else is doing something non-work related.
If all you have to put up with is a little humming count yourself lucky.
