What is your question, exactly? Have we seen any stage screw-ups? Have we been a part of any? Can we think of any? The exact question is unclear.

Perhaps the OP is asking for names of people who are known as 'screw-ups"?
Or perhaps this is a hardware question. For most of our sets, we use Phillips head screws of varying lengths.
