New book about actors who, um, died on stage. I have not read it and can't vouch for it. I don't know what performers are even included. But, here's their website which includes descriptions of performers who, I gather, are not in the book... Show Wont Go On
EDIT: The book is about performers - not just actors.

Less morbid would be a followup tome on actors who have passed out drunk onstage

Posted: 9/4/19 at 6:44pmDue to her back-to-back crazy schedule doing THE GARRY MOORE SHOW and also appearing on Broadway in ONCE UPON A MATTRESS, Carol Burnett once actually fell asleep on stage while pretending to be sleeping on top of the piled up mattresses. The stage manager had to shout from the wings for her to wake up.

That's inevitable - you can't have a book about on-stage deaths without mentioning Tommy Cooper, the comedic magician who died during a live performance at Her Majesty's Theatre in the West End (the one which currently hosts Phantom of the Opera). Yeah, the subject is incredibly morbid, but I suppose someone has to write about it...
P.S Fortunately, we Brits know Cooper for his act as much as his death, and it WAS a really funny act... https://www.youtube.com/watch?v=5yNzkMqBToc
