Caroline is the official standby. She’s the one expected to be at the theatre for every show ready just in case but as a courtesy they’ll offer it to Mandy, depending on the situation.
As I said, in London, other than the preview cancellation, the only other shows Nicole missed were announced a few days in advance. So because the show knew this, they were able to have Rachel in place for those shows. In NY, I imagine they’ll ask Mandy first if they know Nicol

The implication is that Norma does shoot Joe. She then kisses him on the mouth like Salome did which explains why she’s covered in his blood from the mouth down.

I’m not sure of the sight lines at the St James but I found it particularly thrilling up close. Though you probably can’t beat front mezz for the full impact.

The show did 8 performances a week in London Monday - Saturday.
Monday had Rachel Tucker as alternate, Nicole did the remaining 7 shows including the midweek matinee and Saturday matinee.
And it took several weeks for hype to build as word of mouth spread.

The show is not set in modern times. It exists outside of time. All the references to the silent era and DeMille remain alongside some modern references - Betty’s laptop and Norma twerking.

Nicole missed three shows in London. The absences were announced around a week in advance and as such, Rachel Tucker (the alternate) went on for those shows.
Nicole never called out sick on the day.
It was my understanding in London that if Nicole was out, the alternate (Rachel and Broadway’s case, Mandy) had first refusal.
Though obviously if Nicole called out sick with an hour before showtime, the standby would be much more likely as the alte
