Go to the Taxi stand and get a yellow cab. They have a fixed price to Manhattan.

A taxi driver may not know where you are talking about, or where to go. This is usually my experience. When coming and going from the airport, I go with AllState limo, and just order a town car. They are always on time, it's always clean, and these guys actually know where they are going. Good rates too.

Taxi. Normal NYC Yellow cab at the cab stand. Get on the line--it goes quickly. Tell the dispatcher and the driver exactly where you're going. You can't get ripped off because it's a flat fee.
So it's in the driver's best interest to get you to your destination as quickly as possible. The only way they can get more money is with another fare.

And I can't say that I've ever had a problem with cab drivers not being able to find a place in Manhattan as long as you know the cross streets.

A limo (town car) driver doesn't know how to find your specific address any better than a yellow cab driver. And they charge a lot more than a taxi. Last time I took a town car, from either LaGuardia or Newark (I forget which), it cost me around $80. The basic charge, plus tolls, plus tip.
I now take the bus from Port Authority to Newark Airport. It's about $16 one way and they let you off at your terminal. That's it. Takes about 25 minutes.

I'll just third or fourth or fifth what everyone else, except Sutton Ross, is recommending. Take a taxi. Do not fall into the trap of being scammed by a livery (or "black car") service.

Is your destination near a subway station? I've never had an issue taking the airtrain from JFK.
Barring that, taxi is the way to go.
