I use the color shots for everything. If they need to be B&W for something, the graphic designer or myself can easily make it B&W (for a Playbill or newspaper or whatever).

Ozone- with very few exceptions, everyone is going color nowadays in nyc. The real reason for B&W all these years was the processing fee that the repro studios were charging. In the last few years they have drastically dropped there charges for color. I think it has to do with the digital transfer of photos which has cut costs dramatically. For theatre color is now the way to go.
Chris Macke is fantastic and does a ton of Broadway people. His prices are quite reasonable.

The photographer's name is Brian Van Sise. He's a good friend of mine from college. He's won a bunch of photography awards, taken lots of headshots, and has pretty competitive rates compared to other photographers in the city.

Also, I believe that Equity is requiring all actors to make the switch to colored headshots. So, yeah, don't spend your money on black-and-white.

If you want someone cheaper who does more traditional looking headshots, try Nick Granito (http://www.nickgranito.com/). I went to him and my shots came out great.

I second Chris Macke. He did mine and they turned out great. Cool guy too. He made the session alot of fun.
I also believe if you are a student chris macke gives you a nice discount. He is a great guy. He's been doing my headshots for about 5 years now.
Yero- there is no way Equity could make its membership change anything about their Headshot. That's ridiculous!

I think I'm using Chris Macke this up-coming season but a couple of my good friends used Justin Clynes recently and really enjoyed his work. He's extremely reasonable price wise as well.

Definitely pick up a free book at Reproductions and look it over very carefully. I ended up going with Laura Rose who I couldn't have been happier with.

Laura Rose is EXCELLENT! really fresh photos.
DirtySugar is great for more edgy shots, depending on how edgy YOU are.
Robert Mannis, Gaetano Salvadore are good to look into as well.
Peter Hurley is prob the best in the biz, but his rates are totally unreasonable.
Be sure to meet with the photographer before you schedule. You have to vibe with them to get a good shot.

Backstage.com has a forum dedicated to "Headshots and Resumes". I would spend some time there to help you better evaluate your choices.
I'm surprised no one has mentioned Robert Mannis. His are GORGEOUS, thought pricey.
I had mine taken with Robert Wirth, and they turned out fantastic, and they only set me back $400.
