For me it's on the Avenue Q recording during "The Money Song" After Gary Coleman says "Or some puppets will get shot" you can hear this laughing in the background, it sounds like John Tartaglia but im not sure if it is intentional or not.

Thats what I thought but I wasn't sure.
In the OBCR of Beauty and the Beast after "That every daughter's great" in No Matter What there is a weird noise that I can never make out.
SaraBeth: I just listened to my BatB cast recording and for the life of me I can't make out anything unusual. What type of a noise is it?

If I had to describe it, it's a very faint sound. Kind of like a person saying "Ah." It's over almost as fast as it started.

On the Fame Tour Cast Recording with Gavin Creel and Jenn Gambatese, during the end of "Hard Work" there's a sound that sounds like my phone ringing that always gets me.

I love "Fiddler On the Roof," but the OBCR is so hard for me to listen to! The daughters sound awful in "Matchmaker" and Golde isn't any more pleasing to the ear. The only thing I like about the album is Zero, and even he can have his moments of not-so-great greatness.
During "In" on the soundboard recording of "Carrie," it's painfully obvious that many of the girls were taking big breaths to keep up with the crazy dancing, and it's because of this that they are not always together.
It's like:
"I go crazy..."
--"I go crazy..."

Maybe it's just me but there seems to be a volume increase at the end of Rose's Turn with Bernadette Peters and it blew out my headphones once.
Bebe Neuwirth's yuk-yuk-yuk in I Can't Do It Alone makes me want to hurt myself.
Two Gentlemen of Verona--I can't believe they missed this when they remastered the original LP for its CD release, because I catch it every time and it's incredibly annoying. On the track "Follow the Rainbow", there is a point where male voices since "Pool all of our knowledge/keep us safe and sound". Then the group of voices breaks up, and half of them begin singing (in the background) "How can we ever be lost?" Problem is, one of the loud voices from the first line carries over for the first split-second of the word "how", and then is abruptly silenced. Yes, it's only a moment, but it's pretty noticeable.

during Roxie on the Chicago revival cast recording.
I really hate the way she says it. She sounds like she's like the wicked witch or something. But then again that's just me.
"During "Red and Black" on the TAC recording of Les Miz, Michael Ball does this weird embellishment thing at the end of "my world if she's not there." It just seems really out of place."
That has bugged the crap out of me for forever. I'm glad I'm not alone!!

Doubt it. Sounds like a soft chirp and it happens once. If it were a machine, it would happen louder and more often. I mean, yes, I understand the toliet flush in "Mix Tape" that is not in the show, but why would the hospital noises (also not in the show during the song) make it into the recording if they're not necessary? Sounds like a song glitch.
Also, this is not pertaining to any real Broadway CDs, but on the Little Shop Movie Soundtrack I absolutely HATE it that they only leave in parts that are sung (with an exclusion of "Da Doo"), especially ignoring the bridge in "Grow For Me" when Seymour feeds Audrey II his first drop of blood...who's idea was it to take that out?!...
I actually laugh at this moment because it's so bad.
