There's no actual explicit drug use. You won't see a needle go into an arm, and what you do see sure as hell isn't *glorified* drug use. There's no sex -- just kissing. There's no nudity, save for Idina's ass. Those things probably didn't make for an R rating.
Reference to drug use is generally allowed in PG-13. If we were actually seeing someone put a needle in their arm, then that would likely be an R rating. We didn't see that in the stage show, so why would we see it in the movie? Not something I'm terribly concerned about.
Reference to AIDS and illnesses related to HIV are indeed allowed in PG-13. Heck, Philadelphia was rated PG-13.
And insofar as nudity goes, seeing Idina's white butt isn't nudity. As anyone who has ever watched NYPD Blue on primetime TV can attest, showing someone's butt does not mean an automatic R rating.
I really don't think we have too much to worry about, whether MPAA decides to slap a PG-13 or an R on it.

Exactly, there was a booty in Phantom of the Opera and that was PG-13.
"The gods who nurse this universe think little of mortals' cares. They sit in crowds on exclusive clouds and laugh at our love affairs. I might have had a real romance if they'd given me a chance. I loved him, but he didn't love me. I wanted him, but he didn't want me. Then the gods had a spree and indulged in another whim. Now he loves me, but I don't love him." - Cole Porter
If you ask me, I would have rated the musical PG-13...the only thing they probably edited were a few "F" words. It's a rating. I don't get the judgement just on a few letters and numbers.
