Since the Diana only has to sing the score once, in a Studio, how about a dressed down (think Theron in Monster) Catherine Zeta-Jones as Diana. Anyone who's seen pix of her in tabloids, sans make-up, knows she can suggest the demons at work in this tortured woman. She and Downey would be emotionally compelling together. She's the right age, has the chops and actually suffers from the disease. Could be scary good. Diana really has to be a woman in her 40's, or the backstory of young newlyweds makes zero sense.
Madonna for Diana-doesn't she own the rights? Shailene Woodley is a perfect Natalie and she looks like a teen. Downey would be great for the docs. I'd take Chris Isaak as Dan.

Now that Brian D'Arcy James is off Smash, maybe that means he has time to play Dan in the movie. I wish! He and Kate Winslet would actually look good together, like an average suburban couple. With two very famous people I just don't think it would work, but you probably do need a famous Diana.

Diana doesn't have to be that old, 40 would be the max. She's in her late thirties, right? She had Gabe in college (making her no older than 22 at the time) and Gabe is not even 18 yet, in the show.
Toni Collette should play Diana
I feel like the title character of "United States of Tara" is a bit too similar to Diana for me to want to see what Collette would do/ for Collette to want to do it. Though no one has mastered the sad housewife better than Collette as of late.
I just want Anna Kendrick as Natalie. Maybe Ewan McGregor as Dan.
Mark Ruffalo would be a fantastic DAN.

For Diana I'd cast either Carla Gugino, Jennifer Garner, or Christina Applegate. Carla Gugino would make a great film Diana, she has the voice and acting chops equal to Alice Ripley(Too bad Jennifer Connely can't sing, she'd be Diana in a heartbeat)

I can picture Jodie Foster although that may be because of her resemblance to Alice Ripley (at least, I see it)
I love Meryl Streep as much as the next person but she is way too old to play Diana. She turns 63 on Friday....plus I doubt she could sing it even with studio magic and autotune.

The character isn't remotely close to Streep's age. It wouldn't work. I think the Amy Adams idea is quite powerful and astute. I bet she could pull it off, and break hearts. Probably the best idea I've read in this thread.
Nick Jonas would probably get Gabe before they picked a Parrish or Tveit. Especially if it' still a few years off.

RDJ is a pretty good singer
The doctors will be two different actors, I guarantee it. They only use two onstage to further the fact that it is a chamber musical. You can't really make a "chamber film" without playing it for irony (The Klumps, Kind Hearts And Coronets, Austin Powers trilogy), or surrealism (Angels In America, The Cabinet of Doctor Caligari).
Expect an older and well-respected film and television character actor as Dr. Fine, and a star-name actor-who-sings as Dr. Madden.
