I thought The Machinist with Christian Bale was way out there.
Also, someone mentioned the book Beautiful Monster-love that ending.

Psycho: the 1960's version, not the remake.
The Prestige: I loved how the film was basically a magic trick. Chris Nolan is such a great director
Memento: watch it in psychology, liked how it was played backwards and the ending was great
There's an episode of Scrubs that has a great twist ending. The episode has Brendon Fraser return as Jordan's brother, at a point JD tells Dr.Cox that a patient had a heartache and died, and Cox goes into denial and says that it was all JD's fault and stuff like that, so it's revealed later in the episode Brendon Fraser's character was the one who died, even though during the entire episode, he's seen talking to Cox. I love how they make you think it was this other patient who died, but then the big reveal at the end.
Even though I wouldn't consider it shocking, but the original Batman's twist with The Joker being the killer of Bruce's parents was a nice twist.

Defintely Witness for the Prosecution.
I figured out the ending to Sixth Sense, but many didn't. I knew there was a twist to Crying Game before seeing it, so I figured that one out, but might not have if I didn't know there was supposed to be some twist.

Posted: 1/12/07 at 4:53pmI love Harold and Maude. The first time I saw it, I somehow missed her tattoo, so I was knocked for a loop the second time I watched it.

Posted: 1/12/07 at 5:14pmI just remembered one from my childhood. In DICK TRACY I was completely taken for a loop when it was revealed that Blank and Breathless Mahoney were one and the same. As a ten-year-old I didn't see it coming.

The king of shocking endings must be David Lynch. One need look no further than MULHOLLAND DR., BLUE VELVET, WILD AT HEART, etc. to enjoy not only his endings, but his truly unique storytelling devices which are lost on most, treasured and adored by a few.
Since his films are so surreal and mystical (purposely) people are usually so disoriented that they fail to recognize or understand the dramaturgical magnitude of the crazy plot twists that occur. The best example of this would be MULHOLLAND DR. arguably Lynch's masterpiece (well that and BLUE VELVET) which...
****MAJOR SPOILER ALERT***
the entire movie enters a completely different reality once they open the blue box and everyone becomes an idealized or different version of themselves. It's much more complicated than that, but there it is in a nutshell. It's one of the best dramatic tricks I have EVER seen in film. Possibly one of the best twists EVER.
***END OF SPOILER***
I HIGHLY recommend MULHOLLAND DR and most of Lynch's films (DUNE and THE STRAIGHT STORY notwithstanding only because they are examples of Lynch floundering outside his element and being forced to answer to studios).
Also, a hallmark of Lynch films is the integral use of music. In every single one of his films there are at least one or two musical performances and they usually are a MAJOR part of the plot. Angelo Baddalamenti, Lynch's composer of choice (post Blue Velvet), produces the most gorgeous and etheral film scores I have EVER heard. His scores are almost otherworldly, a trait which marries so expertly to Lynch's style of filmmaking. BLUE VELVET is the only film score that can make me weep independantly of the film. It's absolutely GORGEOUS.
So, in closing, LONG LIVE LYNCH!
