Cabin Fever...any of it. Particularly when the dog starts to eat the girl's face in the shed, and you turn and see she's not quite dead yet, but half of the flesh of her face is missing, and you see her teeth through her cheek while the rest of her body is rotting off in gelatinous oozing clots of suppuration.
Mmm. Breakfast time!

Audition is probably the most twisted, sickest movie i have ever seen.
This is lame, I know, but the scene in "Poltergeist" where the maggots pop out of the steak and the guys face melts off in the bathroom. I was a little girl when I saw that movie and it made quite an impression on me.

TheatreBaby, I was going to mention that part of Urban Legend. It was a terrible movie, but the part with the dog really stayed with me. *shudders*
Also, House of 1000 Corpses, the entire movie. I'm still kicking myself for watching that.

the flashbacks in "Stir of Echoes" when the girl is being dragged on her hardwood floor, and is trying to grab at anything, and her nails break off because of the force.

AHHH Pippin! That part with the nail makes me cringe. :[
Also, not really a horror movie, but in "Interview With the Vampire" after Lestat had his throat slit and they threw him in the pond (or whatever), and when he comes back and is all gross and they light him on fire. lol He looks pretty sick/scary.
