I was once in a midtown restaurant, and Idina Menzel was there with her party before a matinee and she was eating a bagel, but just barely picking on it, while they all talked. The entire party got up to leave, and Idina excused herself, doubled back, put the rest of the bagel in her purse, looked at me, put her finger over her mouth and said "Shhhh!" and then left the building. Does that count?
Jeff McCarthy, the original Officer Lockstock in Broadway’s URINETOWN, returned to the role to close the show in 2004.

In Kelli O'Hara's case, I think she left South Pacific due to pregnancy. She was replaced by Laura Osnes. Then when her leave was up, she resumed the role. I believe that when she left the show again, they went back to laura Osnes. Replacing teh same actress in the same role in the same production twice is quite a feat!
Here's one that's a little bit interesting:
Joshua Henry originated the featured ensemble role of "Favorite Son" in American Idiot . He stayed from March 2010 to July 2010 before heading off to The Scottsboro Boys at the Guthrie and then Broadway, where it closed in December 2010. He then return to American Idiot back as "Favorite Son" from March 2011 to its closing about a month later. He received a Tony nomination as Best Actor that season for The Scottsboro Boys.

This is a separate incident. He came back after officially leaving the show when Earl Carpenter was injured. But please, argue with me some more.
