I'm surprised no one has mentioned "Contact" in Rent.
I don't recall Clara being on all-fours in the London production of Passion.
I do recall her having to lift the sheets a bit to give Michael Ball's groupies a brief glimpse of what they came for.
The most explicit production I've seen was a stage adaption of Maurice that toured the UK in the late 1990s. Superbly adapted and acted, the nudity and sexual activity between Maurice and Scudder were not in the least gratuitous but absolutely relevant and necessary, and ultimately succeeded where so many gratuitous productions fail.
In contrast to that I've also seen a recent production of The Romans in Britain whose original staging caused a national scandal in the early 1980s. The play itself was weak in its concepts and funny for the wrong reasons. Thus the notorious Roman-soldiers-meet-the-local-boys scene was something of an anti-climax.

I'm not. I was under the impression the OP was looking for scenes a little more explicit than a bunch of people flapping about under a big sheet.

Because nothing really happens during contact. They just make sex noises and phrases under a big white sheet. If you are referring to the dancers to the side of them, they aren't having sex they are simply doing dirty dances.
>>Avenue Q! Full puppet nudity! <<
Yeah, that is as x-rated as you can get! 10+ positions?
And people had mentioned EQUUS and I'd have to agree because he is bull****ting to the doctor in regards to getting it on with the girl. So she is lying naked and he claims to be thrusting into her, as we can see in front of us.
A play my company is developing called 'Dog Tags' is pretty high on the list when it comes to sexual activity on stage, the 1st scene is a guy taking his girlfriend from behind, both are nude. Later in the show there is a nude scene (just kissing and a bit of groping) between 2 male characters

Puppet nudity in AVE Q? Let's face it neither puppet has any body below the waist. OK so Kate is topless but she's a monster.
I saw the original Campany on Broadway and the touring version and there was nothing going on with Bobby and April in either staging. The emphasis was on the dance that suggested something going on.

How about Captain Jack in MOVIN OUT.
And half of cabaret had graphic sexual acts... remember 'money'... he had cash coming out of EVERYWHERE
Terry and Daisy had intercourse in Side Show during Tunnel of Love, I believe - if I remember correctly
What????
I don't remember that!!
SongAndDance - seems fairly graphic! I love it.

Tunnel of Love was staged rather tamely on Broadway, but yes, it was implied that Terry and Daisy had sex then. Now, when I worked on one of the early regional productions Bill Russell came to see the show. He said our Tunnel of Love was about 100 times hotter than the Broadway version. It helped that it was a much more intimate venue, so you could see just what Terry and Daisy were doing.
Would the rape scene in the Abduction from Man of La Mancha count? Pretty graphic if staged right.
There's the sex scene between Louis and a man he meets in the park in Angels in America (Millenium Approaches).
