To offer a real answer:
None of this has been announced, but the theatre (and actual performance dates) will be announced before tickets go on sale. I would expect them to go on sale by this November.
As for the theatre, there has been speculation it could go into the August Wilson Theatre, Nederlander Theatre, St. James Theatre, or the Broadway Theatre. (The first two seem more likely because of their moderate sizes.)

Here is trip down memory lane
Saw the original with comp tickets via my high school. Papering services did not exist than.Leads were Mini Hines and Johnny Desmond. Hines resembled her more than Streisand .Loved the show and was bummed out the movie left out a few production numbers .Movie was good from a non musical director
