To offer a real answer:
None of this has been announced, but the theatre (and actual performance dates) will be announced before tickets go on sale. I would expect them to go on sale by this November.
As for the theatre, there has been speculation it could go into the August Wilson Theatre, Nederlander Theatre, St. James Theatre, or the Broadway Theatre. (The first two seem more likely because of their moderate sizes.)
Sarge, it will be announced on this very website so just keep your eyes open for that announcement.
