One major change that comes to mind is Mrs. Tottendale in "The Drowsy Chapersone" - Georgia Engels "Bo-Peep" style costume was completely re-designed for JoAnne Worley to more of a flapper look.
Also - when Melanie Brown (aka Scary Spice) did a stint as Mimi in "Rent" - I believe she was one of the few who they changed the typical "Out Tonight" blue vinyl pants for...
Any other major changes for replacements come to mind?

Toni Braxton in Beauty and the Beast. The shortened Belle's blue dress and made it more low-cut. They made Belle a LOT sexier than she should ever be.

When the national tour of Wicked began, Glinda's costumes were slightly changed, all the dresses were shortened and the wigs were made curly and short. They have since returned to the original longer wigs and dresses used for Kristen Chenoweth and Jennifer Laura Thompson.
All of the new Mrs. Wilkinsons in BETM have had different costumes. For example, Hadyn, then Emily, had shiny spandex and leg warmers for Shine, and Kate was more comfortable in the pink and purple dress that they gave her. And they are all altered at least a little bit.
Not Broadway but in London Paulette's costumes changed as a new cast came in:
Almost all of the Nina costumes in In the Heights were changed for Jordin Sparks.
Elaine Stritch's dresses are different from Angela Lansbury's.
The lace Roxie dress in Chicago is sometimes altered for certain actresses. Most notably for Karen Ziemba and Chita Rivera.

Wasn't she? There's a picture of her in the tenth anniversary set.
I know there were changes when LBB left and Baily came in. Most notably the jeans in "Positive" and the skirt in "So Much Better" And Kate Rockwell's wig was very different than LBB's - she had barrettes.

Every actress who played Dolly during the original run of the show had the styles of the costumes modified a bit but they remained true to the orignal's color schemes. The famous red gown remained virtually the same, though.
Billie Joe Armstrong had an entirely different set of costumes for American Idiot than Tony Vincent usually does as Saint Jimmy.

I'd guess more that it was his own clothes.
Chita appeared as Roxie in the Toronto, Las Vegas, and London companies of this production, but has refused repeated entreaties to play the role on Broadway.

Not major, but Meghann Fahy wears jeggings where Jenn Damiano wore normal jeans. Don't know if that was the case when she was the understudy.

Mario Lopez's desire to show off his biceps led to changing Zach's iconic long-sleeved mustard sweater to a short-sleeved brown shirt in the A Chorus Line revival. Nick Adam's Larry had to wear a long-sleeved hoodie in order not to show him up in the guns department.
I think William Ivey Long does a different Roxie dress for nearly every Roxie who comes in--along with the wigs to personalize each woman's portrayal.
Please don't give Long the credit for those atrocious wigs in the revival. He is not in charge of them.

In the original production of Into The Woods, when Phylicia Rahsad replaced Bernie, they changed the "pretty" witch dress and hair. It can be seen by watching the Tony Award performance.

William Ivey Long does do a new dress for every Roxie.
When Shirley Jones played Dorothy Brock in the 42nd street revival she had jackets made for all of her dresses.
All Glindas since Jennifer Laura Thompson have had a shiny sequinned dress instead of the original layers of beaded lace.
Every Elphaba Act 2 dress is slightly different
Is is me or does Phylicia RASHAD look more like her sister in the witch costume in full make-up (in the beginning) than she does normally? Just saying?
