Best use of trap doors on Broadway is currently Fun Home. There are so many and it's almost nonstop, and as someone who loves tech elements, I was in haven.

What about the stunning way that Pride Rock enters in Lion King? I am not sure if it is still done that way with the move from the New Amsterdam, but when I saw it years ago it was breathtaking.

The traps in Phantom are magical. But the best use of traps I've seen are in the current Broadway production of Fun Home.
SPOILER IF YOU HAVENT SEEN THE SHOW:
The one scene at the end where all of Alison's memories start to go away by pieces of the set going under the stage by the multiple trap doors on the stage had me shaking in my seat.

What do you think contributes to a lack of trap doors in Broadway shows?
I think designers are compelled by producers to make shows easily turned into a touring design. Newsies is the first that comes to mind.
Do shows ever create traps because of a lack of storage space for scenic elements that enter via traps?
Are trap doors cost prohibitive or do they come with added insurance requirements? I would imagine that things like Raoul jumping into a trap in Phantom requires extra pay to the actor for the risk.
any insight ?

You mention "lack of trap doors" as though a trap door should b included in a design by default, which confuses me.
There are an infinite number of factors at play.
Broadway stages are bare. There are no traps / elevators. It all has to be designed and engineered and structurally safe for the venue. That requires a lot of work and a lot of money, then you have to worry about the logistics of using it... Getting things on and off, opening and closing - many directors dislike the void created by an open real before and after its usage. It also takes up valuable space in theatre that could be used for musicians, actors, wardrobe and hair... You have to dedicate a certain amount of space for people and things backstage.
When the show closes, you also have to remove everything and restore the theatre... In the case of a trap, that means a lot more work.
as far as recreating for a tour, you would be surprised how often that ISN'T a consideration. But they will find alternate ways to make it happen on the road without a trap, just as if the producer had banished them from the broadway design.

I remember in the original Broadway production of CATS one of the cats (McCavity?) made their entrance by way of a "star trap". It was a neat effect. Here's a video of how a star trap works.

A couple of seasons ago during a Monday performance of Gypsy of the Year, one of the girls from Memphis used the Genie trap door at the New Amsterdam. She was wearing a long sparkly gowns and the train got stuck in the door. It didn't seem like they could open the door again for the train get loose that a stagehand ended up cutting the train to get her free. She look mortified as I'm pretty sure it was a costume from the show. However, IIRC, she still sang throughout this entire ordeal.

I am fairly certain there is hazard pay for such things. Traps can be extremely dangerous. Nancy Opel was injured in one, as was an actor in Dracula - and of course, there was the actor in Spider-Man who suffered a partial amputation of his foot. Trap door automation trumps skin and bone every time).
Because of this, I had a hard time watching Michael Cerveris wend his way around the open trap doors towards the end of FUN HOME. If their intent was to create tension, it worked. For me, for all the wrong reasons - I was personally concerned for his safety. It took me right out of the play.

I remember Lesli saying in her vlogs for Matilda that all the desks and traps had safety and censors. If there's a hand or foot in the way of the desk it doesn't go down. Since there are children in the show it's a bigger deal.

At the LA stop of Jersey Boys, Bob Gaudio fell through the trap. Show had to stop and the understudy took over, if I remember right.
