LOOKING FOR MR. GOODBAR (1977). Truly horrific. It still packs a wallop. Brave, brave performance by Keaton who should have gotten the Oscar for this, not "Annie Hall" I M O...but that's another thread.
The ending of THE MIST is such a gut punch.
Requiem for a Dream.
There are no words.
I know it's ridiculous to say this, but the very last image of THE BLAIR WITCH PROJECT caused me to slap the SH*T out of my friend. I mean...I just turned and slapped him in the face.
Even knowing the twist, I thought Orphan was a pretty disturbing movie.
Planet of the Apes
Yes, the original Planet of the Apes was disturbing to me when I first saw it as a kid.
The ending of the 70's INVASION OF THE BODY SNATCHERS freaked me the f**k out when I was a kid. I still think it's a hugely underrated film.
I also agree with THE MIST. I thought the movie was just ok...until the end. And then I was all, 'The F*CK is wrong with you people????'

A Chorus Line. It's just disturbing that any of those untalented, ugly people would ever be chosen to back up a glamorous star in a Broadway musical.
The Mist
Requiem for a Dream
Boys Don't Cry
Invasion of the Body Snatchers (Sutherland)
Jacobs Ladder
The Wicker Man (original)
Se7en
Seconds.
^^^^^
Oh god yes.
If not disturbing then undoubtedly the most disgusting. ugh.

I'll second SECONDS.
And I'll add THERE WILL BE BLOOD, and SALO.
SE7EN came to mind first, even though it totally is consistent with that film, as it was such a brutal gutpunch.
And not that it's the spirit of the question because the whole movie is "disturbing" to great comic effect, but MAN BITES DOG ends the only way it possibly could and is amazing. I love that ending.
Speaking of foreign films from the mid-'90s: LA HAINE. Holy s#!t.
Looking for Mr. Goodbar is mine as well. I could never EVER watch that again.
The ending of The Mist just pissed me off because I loved the ending of the novella.
I feel like I'm the only person who could just barely get through THERE WILL BE BLOOD. I tried watching it again after it came out on video but after 30 minutes I realized that I just really hate that movie.

You're not alone -- lots of folks hate THERE WILL BE BLOOD. I got it from the first time I saw it, and repeat viewings have only increased my affection and respect for it.
^ I hated THERE WILL BE BLOOD with a passion that burns with the fires of 10,000 suns.
That f*cking milkshake line made me want to kick a toddler in the face.
Carrie
And these weren't the final shots, but they were the endings/conclusions of the films:
Don't Look Now
Spitfire Grill
I absolutely LOVE There Will Be Blood. I've seen it several times now, and it gets better with each viewing.
I don't just think it's good, I think it's a masterpiece.
Thank you Roscoe for reminding me of SECONDS.
Also the 1978 remake of INVASION OF THE BODY SNATCHERS.
One I doubt many of you have seen, but Gaspar Noe's Into the Void has haunted me since I saw it.
Also, Von Trier's Dancer in the Dark.
I'm listing the endings I found disturbing, not just twisty.
