I don't know if this counts but the two actors I saw in the North Shore Music Theatre's Production of Kiss Me, Kate who played Lois and Harrison were absolutely terrible. Lois could not act and Harrison played the role straight. He didn't play the role like some macho mysoginist schmuck. His performance was very, "I think I've got a valid point to make." And I sat there thinking, "Huh, I see what you mean." In other words, he made Harrison far too sympathetic. They were cracks in what I thought was an otherwise gem of a production.
Susan Egan in anything at all. She's not very good at all. IMO
Kate Loprest as Glinda in Wicked. SH*T ABSOLUTE SH*T.
Hasselhoff in Jekyll and Hyde...wtf?
Patrick Swayze in Chicago
Loved Patti Cohenour as Margaret in Piazza. Not as powerful as Clark in the end, but often more moving along the way.
Rathnait62... No, I did not see Peters live only on video. And eventhough I think Peters was better during Stay With Me and maybe Lament, Vanessa Williams really blew me away in the second act. Her transformation was very believable and her Last Midnight is amazing.
Kerry Butler in Little Shop, Lauren Kennedy in Spam. Many others have already been mentioned, no need to repeat.
daphne in les miz
I though kerry butler was fine in little shop considering ellen greene is inimatable.

Katie Clarke as Clara
Patti LuPone as Fosca - gonna get shot for this, but hey
Michael Cervaris as Giorgio - come on, Giorgio's supposed to be this hot young boy toy, plus I'm not big on his voice period.)
Idina Menzel as Amneris - honestly, who bought it?
Christy Carlson Romano as Belle - though that's being nice, honestly.

Not to be terribly snakry, but SporkGoddess, isn't that Katie Clarke in your avatar? (I love it though, it is a nice picture)
I actually enjoyed Menzel's Amneris...she was just ditzy enough to work, without going total airhead.

Colm Wilkinson in Phantom (yes, I realize he was originally the first choice for the part) and Les Mis (why is he on every English cast recording but one?). I really don't understand what people see in him. It distresses me.

Joey Fatone in Little Shop of Horrors revival
Richard Kind in The Producers
i thought joey fatone did fairly well as seymore. although i will say that james moye as orin on tour was truly lackluster
Betty Buckley was a very odd Peter Pan.
While there are many roles I wished were being done by someone else, I'm not sure I'd say they were miscast. Only 2 come to mind that I saw:
Liza in V/V
Glenn in SUNSET
Luker in TSOM
In films, there's a completely different list.
Blythe Danner in 1776
Barbra Streisand in HELLO, DOLLY!
Anne Reinking in ANNIE
Audrey Hepburn in MY FAIR LADY
Cyd Charisse in BRIGADOON
Albert Finney in SCROOGE
Barbra Streisand in ON A CLEAR DAY YOU CAN SEE FOREVER
Lucille Ball in MAME
Carol Burnett in ANNIE
Matthew Broderick in THE MUSIC MAN
Glenn Close in SOUTH PACIFIC
Mitzi Gaynor in SOUTH PACIFIC
Rossano Brazzi in SOUTH PACIFIC
Bette Midler in GYPSY
Jason Alexander in BYE BYE BIRDIE
Chynna Phillips in BYE BYE BIRDIE
Rosie O' Donnell- Every Musical she Ever Did
My sister is 3, and she loves Grease, But she refuses to listen to the revival cast because of Rosie's voice
and I saw her is Fiddler, It hurt

Vanessa Williams was dark and haunting in the role of the witch in ITW because she was HORRIBLE in the role. She can't act. AT ALL... the rewrites they did for her for Last Midnight were PAINFUL to watch.
Funny enough... Bernedette (who was an amazing witch) was TOTALLY miscast in Gypsy.
Charlotte in ACL, loved her in Chicago, hated her in ACL.
Yeah, um I didn't care for Richard Blake in Wedding Singer. He had the voice and the look but he wasn't quite smarmy enough. He could have been a bigger jerk.
And I HATED Duff in Hairspray... Yuck.
"If we don't live happily ever after at least we survive until the end of the week!" -Kermit the frog "I need the money... it costs a lot to look this cheap!" -Dolly P. "Oh please, Over at 'Gypsy' Patti LuPone hasn't even alienated her first daughter yet!" Mary Testa in "Xanadu" "...Like a drunk Chita Rivera!" Robin de Jesus in "In the Heights"
"B*tch, I don't know your life." -Xanadu After that if he still doesn't understand why you were uncomfortable and are now infuriated, kick him again but this time with Jazz Hands!!! -KillerTofu
Keith Carradine in DIRTY ROTTEN SCOUNDRELS
The entire cast of the First National Tour of TITANIC. My ears were bleeding during that performance.

Kerry Butler in Little Shop
I think she is a talented performer, but I just can’t help but see "Disney princess" in anything she dose.
I was fortunate enough to see Alice Ripley do Audrey in Miami and thought she was absolutely incredible.
Jason Pebworth as Raoul. Eeugh. Balding, no substance to his voice, completely not charming. Or even interesting.
"Patti LuPone- Passion "
Seriously?
I don't chime in too often but here are a couple I would have to mention...
Karen Ziemba - Curtains
Dawnn Lewis - Sister Act the Musical
Kerry Butler - LSOH
Bernadette Peters - Gypsy
Debra Monk - Company
Audra McDonald - Dreamgirls (I know it's only a concert, but her legit voice drives me batty over pop arrangements)
Heather Headley - Dreamgirls (again^)

""Patti LuPone- Passion "
Seriously? "
Absolutely. Granted I'm biased because I don't like her generally but she was totally wrong for Fosca IMO. I adore that role, Donna Murphy was beyond brilliant, and Maria Friedman was excellent. To me Patti LuPone couldn't match Donna understated, subtle performance or Maria's over the top interpretation. She went with a halfway thing that didn't come off for me. Plus I can never understand what she's saying and with Sondheim the lyrics are important to say the least!
