I agree with madison2: Brian Stokes Mitchell received one after that song both times I saw the show. He was good, but I remember the first time thinking, "What are these people doing?!?" But within seconds, the entire orchestra was on its feet.
Hugh Jackman at the end of his 11 o'clock song "Once Before I Go" and at the last revival of Hello, Dolly, the title number got one.
Yep, I've seen it -- during Sound of Music. The first act closes with "Climb Ev'ry Mountain" and Patti Cohenour brought the house down...it's the first & only time I've seen it and experienced it!
In 50 years of theater-going, I've only seen it twice. Both shows it happend more than once. In the original London production of My Fair Lady, the entire audience lept to their feet after The Rain In Spain, I Could Have Danced All Night and Get Me to the Church on Time. During the Broadway production (and even road production) of Seesaw, everyone stood after Chapter 54, Number 1909, the three times I saw it on Broadway and the two times I saw in on the road. During one of the road performances, they also stood after It's Not Where You Start.

I'll never forget when I saw PURLIE , Melba Moore sang I GOT LOVE and stopped the show cold. Two things happened that had never happened before to me at any show. First , not only did she stop the show and get a standing ovation but I saw people STANDING ON THEIR SEATS and screaming Bravo. Second , after the ovation finally died down....she did the whole number another time. It was pure magic!
The only time I've seen this happen was when I saw "Jersey Boys."

It was truly an amazing evening when Raul sang Being Alive at the first performance of Company following the Tony awards. Everyone was on their feet for several minutes and he was obviously quite moved by what was taking place.

I'm pretty sure some people (ha! I'm listening to that song as I type...) stood up after 'Everything's Coming Up Roses when I saw Gypsy last month. I would have but I couldn't move, being so awe-struck.

Funny enough the only time I had experienced a standing ovation in the middle of the show was when I was at a performance of the National Tour of Brooklyn starring Diana Degarmo in San Jose. It was crazy, I believe she received one during Once Upon A time and the largest one during Streetsinger which almost stopped the show cold. It was so crazy and she was hella good.
As someone mentioned, during Spring Awakening (in previews) after "Totally F*CKed." The bows were equally hilarious, because either they didn't have them firmly choreographed yet or they had configured them for less applause than they received, so you could see the company trying to figure out whether or not to come back after their first group bow as they left the stage. (They did, of course.)
Clay Aiken got one nightly for You Won't Succeed On Broadway.
I think the only time I remember that happening was after "Give Peace a Chance" at Lennon which is the last number in act 1.
Never seen a mid-show standing O. I'm hoping I'll see some Sunday night at Spring Awakening.

Christine Ebersole got one after "Revolutionary Costume" many times. I was there for a couple of them.
Beth Leavel got one for "As We Stumble Along" in THE DROWSY CHAPERONE at least once (full house; excuse the pun)...and some people were standing for her this past Saturday night right after her "Where Has My Hubby Gone Blues" in NO, NO, NANETTE.
^^ Beth got one at the last show, I was there and it was amazing, she held her note for like 5 hours. It was really incredible.
