Every single Fantine wig up until the late 90's. A notable exception is the one Christy Baron wore when she played the role on Broadway in the early to mid 90's. Other than hers, the 80's and 90's weren't kind to the jilted French maiden's hair.

The Glinda wig tends to change for each actress. Sometimes they are longer. (Kirstin Chenoweth and JLT) But others it can like Stacie Morgain Lewis' wig and it just looks like a cap with curls on it.
Rapunzels wig in the original ITW. The color was far too blonde for the skin tone of the actress.

The wigs/facial hair in the London production of Lord of the Rings were pretty god awful. Especially the doorkeeper's in act one before "The Cat and the Moon."

Dreamgirls! The girls wigs are pretty bad. I thought they looked pretty bad from a distance, but a friend of mine works in the hair dept. at the theatre. And when the show came to town I got to go back stage and see them. They looked a hot mess!!!! Updated On: 8/2/10 at 04:07 AM
In the filmed production of Sweeney Todd with George Hearn and Angela Lansbury, Johanna's wig was disgusting! Then again, she was kind of outrageously bizarre in general.
