Nikki Snelson from WONDERLAND (Guess she actually should count it as a blessing), in order to bring Linda Eder on board (who didn't want to do the Tampa 2011 "try-out". Kate Shindle eventually got the role.
Stark Sands was not fired from Bonnie & Clyde - as mentioned, he was involved with American Idiot (already on Broadway), though I think the critics/audience agree Jeremy was better. Stark has performed at a few Frank Wildhorn & Friends concerts since.
Not mentioned before in this thread, I don't think -- Sebastian Arcelus was promoted to full-time Roger in Rent after Manley Pope left, and then abruptly demoted.

Carolyn read as older than the other three leads, which she indeed was.

It makes sense I guess. One would think they would have noticed that when she was cast. Did anyone see the previews of guys and dolls? How was Carolyn?
One of books of Broadway anecdotes recounts the story about Carolyn Mignini being let go from GUYS AND DOLLS. Carolyn and Jerry Zaks had appeared as performers together in "Tintypes" and she was a sentimental favorite of his and he cast her against best wishes of the producers who thought she wasn't a good fit. It became clear during previews that the performance wasn't working, particularly with the much younger Peter Gallagher as her love interest and so Zaks gave in, acknowledging that her dismissal was entirely 'his fault' as a director.
That's more or less the story as I remember reading it.

They probably did have some idea she was older or looked older when casting her, but things change. You may think "oooh it'll be interesting if Sarah is much older" but when you see it on stage it may not work.
Not saying that's what happened, but things like that are often the case and it's not always the performers fault.
Yes - I don't think her dismissal really had anything to do with Carolyn's skills or abilities.It sounds like Jerry's judgement was just clouded a bit by his friendship during the casting process. That revival was also 'in the works' for many years and so its possible that Carolyn was attached (at least in Jerry Zak's mind) for a long time and by the time the production was up and running and the other pieces had fallen into place, she just wasn't the right casting choice any more.

James Weissenbach was replaced by Jim Walton, yes. Jim was playing Jerome the lawyer but wasn't covering Franklin, though, David Cady was. Guess the producers just had a hunch Jim could do it.
Andrew Lloyd Webber saw Martin Smith (a replacement for the title role in the London production of Phantom) and basically fired him during intermission.
Another Phantom, a standby named Nigel Richards, was fired for not listening to the resident director who had curious ideas like "The Music of the Night" being a music lesson... that resident director has since been responsible for the new touring productions of Phantom, Les Mis and Miss Saigon. Another Phantom directed by him left the new US tour under suspicious circumstances.
Actress Melissa Errico was replaced in the role of Clara in the 2013 off broadway revival of PASSION.
I remember there was some back and forth between Errico and the Classic Stage Producers with saying something of their side of the story. Rebecca Luker stepped in last minute to record the cast recording.
Wasn't there stories that a couple were replaced or some left during Spider-Man Turn Off the Dark?
Could be wrong here I seem to recall Joan Jett being replaced in the Broadway revival of Rocky Horror Show too.
The whole thing with Errico and CSC is well-documented on Errico's blog, or at least her side of it is. From what I gathered, she missed a lot of performances due to illness and vocal problems, and her contract stipulated she could only miss a certain number of performances during the run and she was let go.
When Spider-Man was rewritten, the actors playing the "Geek Chorus" left the show as the characters had been cut entirely. T.V. Carpio, who had originally been part of the Geek Chorus, had already replaced Natalie Mendoza as Arachne.
Also, wasn't Joan Jett not allowed to participate in the Rocky Horror cast album due to a conflict with her record label? I don't recall hearing anything about her being fired.
I remember seeing Joan Jett first time I went on see Rocky Horror Show.
I remember getting a cd at the theater for free promotion of the songs. Later on the cast released the cast recording and on my second trip I got my recording signed by the cast except Joan Jett who had left the production was replaced by her understudy who sadly I've forgotten her name but I do remember her being quite good.
Another time I went to see the show to find Dick Cavett, Lea Delaria, Alice Ripley, fine actor who played Brad were not there. Actors I saw that I remember were Ana Gasteyer as Colunbia; an ensemble actor in the role of Eddie and Dr. Scott. Sally Jesse Raphael as Sally Jesse Raphael and Luke Perry as Brad Majors (a**hole) hahahahahahahahahahaha Brings back some great memories :)

Joan Jett left RHS for the same person who made Sherie R. Scott's life utterly miserable during ReNt: Kristen Lee Kelly.
