I know of one HUGE mistake, though not on B'Way. During the JCS revival in London, I think, the man playing Judas accidentally actually hung himself. Suck-tastic.

It was actually in a Greek production of JCS, but it really did happen - horrifying!

When I saw "I Love You Because" Stephanie refered to "Jeff" as "Austin" I guessed cause David's last name is Austin.

one big obvious mistake happened when "Wicked" opened with Kristin Chenoweth still cast as Glinda and it continued until she was replaced by Jennifer Laura Thompson.

Ok I have 4 for you
1) Hairspray november 2004, during I Can Hear the Bells, one of the girls by the organ pitched her arm back with the bell, but it went flying out of her hand and into the wings. She went through the rest of the motions miming a bell. When they circled the stage in single file an arm reached out and handed her back her bell. I still find that one of the funniest things. Spotlight on her along with the *ding* from the pit and the bell soars upward.
2) In January 2005, I was at RENT and Drew Lachey tripped not once, but twice in the first 10 minutes. First he tripped stepping off of the chair, but played it off cool. However you can't really come back after stumbling over the power cord. He can still do all that though and manage to impress me more than the 3 times I saw matt caplan.
3) At The Color Purple, Kenita couldn't pull the letter off the string and ripped it in half
4) During the Lincoln Center Archived performance the wednesday before Pacific Overtures closed, a man who spun the sword around in the opening Advantages of floating in the middle of the sea dropped the sword. Ok it wouldn't have been so bad....except the stage is surrounded by a real waterbed. Oops. He sort of looked a little puzzled for a moment then in one motion swooped his arm down and with the other holding his sleeve, reached deep into the water and grabbed the soaking wet sword. Yeah have fun watching that tape lol

I wish I'd seen this, but apparently 10 years ago when Ricky Martin was Marius on Broadway, he sang during "Every Day," "and I'm Cosette" instead of "Cosette, Cosette!"

When I saw Avenue Q, the "Kate's Monster School" sign didn't properly open. It was covered later, when Brian (Jordan Gelber) made some comment to Kate about having enough money to buy a new sign. They finally unfurled it during the curtain call.

Seccond time seeing Wicked: when G(a)linda (Megan Hilty) came out of her bubble and said "...and we were both very young " (During Dear Old Shiz)Then Shoshana Bean runs in (in case you haven't seen it: she's supposed to run in),and she KEEPS running, and i'm like 'wow, she's gonna run off the stage and into the orchestra pit.'And she DOES. She was fine but i was still laughing histerically throughout the entire show.

Wicked x3, didn't Shoshana then fall into the pit ontop of everyone? Cause I remember hearing this story and then after sitting in the lotto seats a month ago, I check to see and I didn't see a net of any sort.

Yea, no net for wicked, so had she fallen she would ahve fell about 8 feet and onto a violinist and then to a concrete floor. kinda strange.

When I saw DRS at the end, Norbert started to put on his hat and it fell off...John made some crack about it and they both started laughing. The audience applauded..hahah.

The first time I saw the Producers in London, after Springtime for hitler finished, the german dancers drop came down and just stayed down for a very long time...then the orchestra ran out of music to play because whatever was wrong was taking so long and once that had stopped you could here the movement and lack of movement of the set behind the drop. There was silence for a good three of four minutes until the set of Max's office came out again.
I think I've read this has happened before and in that instance the wait was much longer and the actor's came out to talk to the audience.

she kinda hit i think a violinist or someone who was playing an instrument with strings, but she landed safely on the hard floor. But shoshana's really good at making it seem funny, so i couldnt hear what she was saying because i was laughing too hard, she said something that made the whole audience laugh.

I saw Wicked on Feb 4th. During Thank Goodness, when Glinda calls after Fiyero, Megan called him Derrick. My friend didn't notice, but I kind of looked around to see if anyone else did. I'm not sure.
